  • You are the birth parent or legal guardian of a child with an intellectual or Developmental Disability

  • You live in Staten Island with your child

  • Your child meets OMRDD’s criteria for developmental disabilities’ services (has a recent psychological evaluation with IQ and Adaptive Behavior Scale – Vineland II – scores)
